Health Advocate Coordinator
The Health Advocate (HA) Coordinator is responsible for coordinating and supporting Health Advocates. Their goal is to ensure that Health Advocates are having enriching experiences in Health Bridges. 

Duties and Responsibilities
  • Contribute to a weekly 1 hour meeting with the HBLT
  • Track Health Advocate attendance at shifts, trainings, and other events
  • Schedule a 10-15 minute 1:1 check-in with each volunteer per semester
  • Manage email communications/reminders to Health Advocates
  • Coordinate hospital shift schedules and troubleshooting
    • Read weekly reflection forms and report problems to the HBLT
    • Read screening forms to track patients’ social needs
    • Maintain the Follow-Up call spreadsheet and translation and community resource handouts
    • Provide regular technical assistance to Health Advocates during their shifts at PVHMC
  • Manage Health Advocate/PVHMC Improvement Projects
    • Set deadlines for projects (proposal, finished product, etc)
    • Obtain regular updates and update the HBLT on ongoing projects
    • Perform data analytics on HPE and in-patient interactions
  • Event-Planning
    • Propose ideas for Community Building Days & workshops
  • Provide additional support to the HBLT and other coordinators

Qualifications
Required
  • Currently enrolled as a student at one of the Claremont Colleges*
  • Commitment to 2-6 hours per week for a minimum of 2 semesters
  • Understanding of and dedication to Health Bridges’ mission
  • Effective oral and written communication skills
  • Strong organization skills, timeliness, attention to detail, and ability to manage multiple projects simultaneously
Preferred
  • Prior PVHMC shift experience
  • Flexible schedule to accommodate weekend and/or evening communications
  • Interest in long-term commitment to Health Bridges
  • Strong interpersonal skills, resourcefulness, and optimism
